Run personal-injury notes through an AI agent that lists what to fix before they go to the lawyer

who Dr. Corey Malnikof, who runs Palmer Care Chiropractic (24 clinics and about 150 team members, based in Sterling, Virginia), on The ChiroX Show podcast

What it does

The group built an AI agent that reads the PDF of the personal-injury notes and returns a list of what needs to be fixed before the notes go to the lawyers. Separately, it is finishing a project to auto-post EOBs (insurance explanations of benefits) from all its clearinghouses into its EHRs, with a report of what's going on and what needs to be done next. He doesn't name the tools.

Tool cost

The problem

Personal-injury notes get extra scrutiny once they reach the patient's lawyer, so they need a fine-tooth-comb review before they're sent, across 24 clinics.

What happened

He calls the notes agent "so freaking cool" and expects the EOB auto-posting to "take the job of like 10 people and turn it into one" (an expectation, not a measured result). He adds that the AI phone companies the group hired had more hang-ups than good conversations, so AI doesn't answer its phones yet. Self-reported.

How to set it up yourself

  1. What Palmer Care Chiropractic does: before personal-injury notes go to the patient's attorney, staff run the PDF through an AI agent the group built, which lists what needs fixing; a person makes the fixes. The tools aren't named, so the steps below are a DIY version.
  2. These are patient records, so use only a setup your practice has a BAA for or remove identifiers first; some EHR and AI companies sign BAAs on business or enterprise plans, so ask. Check with whoever handles your compliance before the first real note goes in.
  3. Write down what your PI attorneys and insurers push back on (missing dates of injury, mechanism, functional limits, objective findings, plan and duration, signatures). Put that checklist and the prompt below in a Claude or ChatGPT project's instructions.
  4. Upload each PI note PDF to the project before it's sent, fix what it flags in your EHR yourself, and keep a log of what it catches so you can tighten the checklist. The doctor still signs off on every note.

Copy-paste prompt

Review this personal-injury note PDF from {PRACTICE NAME} before it goes to the patient's attorney. Check it against our checklist: {CHECKLIST, e.g. date and mechanism of injury, history, subjective complaints with pain scales, objective findings and measurements, diagnosis, causation statement, functional limitations, treatment plan and duration, progress compared with last visit, provider signature and date}. List every missing, inconsistent or unclear item with the page and line, and what to add. Don't rewrite clinical findings or suggest findings that aren't documented.
